I represent a healthcare, community-based organization or legal/financial service provider and want to participate with AzHDR.
Examples include, but are not limited to the following:

Healthcare – Primary care physician, behavioral health provider, hospital
Community Based Organization – Faith based organization, housing center, school, career center
Financial – Wealth manager, bank, fiduciary
Legal – Law office, ombudsman, mediator

Participating Subscribers

AzHDR Subscribers are qualified organizations that are granted access to the secure website.

Eligibility

Eligibility is reviewed during the initial call with an AzHDR team member. This call will determine the type of access an organization has in the portal.
