This compact trifold brochure is designed to help individuals understand:

• The benefits of registering advance directives with the new AzHDR
• Why advance directives are important
• What forms are included and/or required
• How to get started

All too often, individuals put off addressing advance directives because they are overwhelmed by difficult-to-understand, dense information and instruction.

In this brochure we have attempted to uncomplicate the process and give a succinct, clear overview— while directing readers to AzHDR.org for more information and resources.

Files are ready for you to download and supply to the printer of your choice. Fold marks have been included on the file.

• Flat size (unfolded): 11” wide x 8.5” tall
