AzHDR.org – Home of the Registry

The Arizona Healthcare Directives Registry (AzHDR) now resides with the Arizona health information exchange (HIE). In 2019, legislation authorized the transition of the registry from the Secretary of State’s office to the HIE to improve healthcare provider access to advance directives.

In 2019, Arizona Senate Bill 1352 was passed (statutes 36-3291 and 36-3295) giving healthcare providers the ability to have real-time access to patients’ wishes – when they need it most.

AzHDR.org is designed to help honor your end-of-life healthcare wishes by providing secure and seamless access to advance directives, documents that outline your healthcare preferences, across the continuum of care. The AzHDR provides a safe place to store and make accessible Arizonans’ advance directive documents so end-of-life care will be guided by their wishes.

Registering advance directives with the AzHDR is free to Arizona residents and offers easy access to participating healthcare providers – ensuring wishes registered are wishes honored.

Working together as a community, we can ensure that no matter where or when someone faces the moment they can’t communicate, their advance directives are registered, accessible and honored.
